In choosing to participate in athletics, a student agrees to abide by not only the policies listed in the Student Handbook, but also additional policies, including, but not limited to, the policies of his/her coach(es), the policies listed in the Athletic Handbook, and the policies of the Illinois High School Association (IHSA). (The IHSA policies include, but are not limited to, those provided in the “2019-2020 Key Provisions Regarding IHSA Rules”.) The Administration will ensure that all additional policies are consistent with the philosophy of Mater Dei.
In addition, for each school year, prior to a student’s first day of practice, he/she must submit an “IHSA Pre-participation Examination” physical form. NOTE: Incoming freshmen students, for whom a physical is required by the Illinois State Board of Education, are not required to also submit the IHSA physical form.
Finally, for each school year, prior to a student’s first practice, he/she must submit an “IHSA Sports Medicine Acknowledgement and Consent Form.” NOTE: This form is submitted electronically via our 8to18 website.
